Our team conducts a thorough assessment of your condition before creating a care plan to ensure we address your needs early on. We work with you to identify any necessary equipment and services that may be beneficial to make the transition as easy as possible. Our Therapy Department evaluates your home and provides suggestions to make the environment safe for your return. The Social Service Department schedules a discharge meeting with you and your family, providing guidance on how to access resources and support, and offering advice on managing any challenges.

Safe Return
Therapy Department
Our Therapy Department will make sure you are ready to go home by offering an in-home evaluation. They will assess your home and make suggestions to make the environment safe for your return.
Social Service Department
The Social Service Department will schedule a discharge meeting with you and your family. Those involved in this meeting are the Therapy Manager, Unit Manager and Social Worker.
